Kandu Health, an Imperative Care business, provides stroke survivors with the guidance they need at home with a 90-day virtual recovery program.  We partner with hospitals to ensure patients have a safe transition home and a customized recovery plan. Together, we provide continuity of patient care to achieve better quality of life for stroke survivors and their care partners.

Reducing Disparities in Access to Rehab through Digital Health Applications

Stroke rehabilitation begins during acute hospital care and continues throughout the lifespan for many individuals who have sustained a stroke. Access to stroke rehab is limited by funding, location, and culture– causing disparities in access to care.

Social Determinants of Health (SDOH) are a significant contributor to disparities in health and have been demonstrated to put individuals at higher risk for 90-day mortality. Understanding and addressing SDOH needs after stroke can support improved health and outcomes as survivors return home.
